To be added to our waiting list, please complete this online application. Upon acceptance a non-refundable deposit is required in the amount of $300. The deposit can be mailed to Colleen Bowman or payment can be paid via PayPal (no account is required) Payments made via PayPal will have an additional 3% fee. Deposits are applied to the purchase price of the puppy. Please read and understand the Puppy Contract here. Puppies being purchased for pets are required to be spayed or neutered, please read the Spay / Neuter Addendum. Purchase price does not include shipping.

	I have answered all of the questions of this questionaire truthfully. I understand that if any statements are determined to be false, McKenzie Crest GoldenDoodles reserves the right to NOT sell to me and I will forfeit all deposits. I also agree that if it is determined that I can no longer care for the dog, it will be returned to McKenzie Crest GoldenDoodles. Balance of all monies are due in full 6 weeks from the date of the birth of puppies. The puppies are usually ready to go to their new homes at 8 to 9 weeks of age.
